There is currently a known issue where destination islands sometimes do not spawn, even when your raft is right on top of them until you restart the game. This might result in your raft getting stuck below them.
If you find your raft stuck under an island and you have auto-save enabled you can use the backup system to get out. You find your backups by selecting your save in the load world menu and clicking the world backups link.
